چکیده

The key therapeutic intervention during endoscopic retrograde cholangiopancreatography (ERCP) is sphincterotomy, but lack of training and experience leads to a higher risk of complications [1,2]. Herein, we present a simple, reproducible, easyto-build, ex vivo, ERCP model for the training of sphincterotomy using standard sphincterotomes and a needle knife. The model is based on incorporating one or more chicken hearts into a pig stomach (●" Fig.1,●" Fig.2,●" Fig.3,●" Video 1). The native pig papilla has some limitations for sphincterotomy training, mainly because of its awkward location in the bulb compared with in the second duodenum in humans. To solve this issue, we previously created a model in which the pig stomach was “duodenalized” to resemble the duodenal sweep in humans [3]. The duodenalization overcame the common position problems when performing ERCP in intact pig stomachs, which are large and J-shaped. In a further development of Fig.1 Chicken hearts serve as ideal models for papillae. Their shape and anatomy resemble true ampullae. a The ventricle and aorta are perfectly suited to create a “bile duct” channel. b The chicken hearts can be cut in half to create smaller “papillae.” c The ventricle can be punctured to create a papillary orifice. d An implanted papilla. e Stent inserted into implanted papilla.

A novel mechanical simulator for cannulation and sphincterotomy after Billroth II or Roux-en-Y reconstruction

INTRODUCTION In patients with Billroth II (B II) or Roux-en-Y anatomy, endoscopic retrograde cholangiopancreatography (ERCP) is demanding. Here, we describe a novel simulator with simulated fluoroscopy for cannulation and sphincterotomy training in such situations. METHODS A custom-made simulation system was built based upon a common chassis of a series of previously described ERCP simulators...

Endoscopic papillary large balloon dilatation with sphincterotomy is safe and effective for biliary stone removal independent of timing and size of sphincterotomy

AIM To describe the efficacy and safety of endoscopic papillary large balloon dilatation (EPLBD) in the management of bile duct stones in a Western population. METHODS Data was collected from the endoscopic retrograde cholangiopancreatography (ERCP) and Radiology electronic database along with a review of case notes over a period of six years from 1st August 2009 to 31st July 2015 and incorpo...

Precut sphincterotomy for selective biliary duct cannulation during endoscopic retrograde cholangiopancreatography

Selective biliary cannulation during endoscopic retrograde cholangiopancreatography (ERCP) is required to perform all therapeutic biliary procedures. Despite major advances in imaging, guidewires and sphincterotome catheter designs, the success rate for biliary cannulation by experienced endoscopists during ERCP is approximately 85% when standard cannulation techniques are applied. Precut sphin...

Recombinant Factor VIIa Use for Endoscopic Retrograde Cholangiopancreatography With Sphincterotomy in a Patient With Choledocholithiasis and Unusual Coagulopathy

Endoscopic retrograde cholangiopancreatography (ERCP) is a procedure that combines the use of endoscopy and fluoroscopy to diagnose and treat pancreaticobiliary disorders. The risks of ERCP include pancreatitis, infection, bleeding and perforation. Bleeding during ERCP typically develops after sphincterotomy, hence patients should be screened and tested for coagulopathy before undergoing ERCP. ...

Competence development in ERCP: the learning curve of novice trainees.

BACKGROUND AND STUDY AIM Measures for competence in endoscopic retrograde cholangiopancreatography (ERCP) during training are poorly defined. Currently, various training and accreditation programs base verification of competence on performance of a minimum number of procedures. There is a general awareness that procedural competence certification should be based on objective performance criteri...
